MK5 Shallow anchor design is based on the bolt located between at least 1 upper beam bar and lower than the slab grid bars at center line slab thickness, surrounded by high yield post-tension bar.
Extra heavy anchors used on the MK10 and MK12 foundations use an 8” square steel plate holding 4 anchor rods sitting on top of the bottom beam bars held in place with the anchor bolt location template.
